Pharmacy Database. I need a program designed to the following requirements.The program needs to contain 6 inputs, one of which will be a free input to allow an address to be added. The other 5 inputs will contain drop down lists of 10/30 variants and 3 of these input will need to be multiple entries.

When the data has been inputed it needs to be written away as a read only file (no modification to any inputed stored data is allowed once stored) and be accesible on a read only basis by day/week/month preferably in a PDF for ease of printing hard copies and emailing.

The program needs to be well presented and windows orientated with drop down lists etc.Full analysis will be provided for ease and speed of completion.
